 Responsibilities: Maintains clinically competent in providing direct patient care, evaluates outcomes, consults with other specialists as required and adjusts nursing care processes as indicated to ensure optimal patient care

Coordinates patient care through collaboration with the patient, family, all other disciplines, primary and secondary physicians and community resources/programs

Demonstrates knowledge of Hospice philosophy and provides support to patients and their families Ensures that patients understand their rights and responsibilities, advanced directives and the meaning of informed consent during the admission process

Assesses and directs patient and family teaching needs and is proficient in the use of return demonstration and teaching aids in the home setting Maintains professional responsibility for knowledge of current nursing research, practice standards, guidelines and protocols, and hospital policies, procedures, and programs to improve patient care quality

Maintains proficiency in the use of all equipment used in his/her practice Effectively communicates and assists other members of nursing team and/or other disciplines

Actively participates in problem solving and conflict resolution to maintain team cohesiveness Performs on-call duties effectively with the ability to triage patients over the phone and visit when needed

Directs own educational needs for the top 10 ICD9 codes and/or diagnoses for SGH HHC: CHF, COPD, diabetes, wounds/post-surgical, arthritis, newborns, Coumadin dependency, malnutrition/ nutrition needs, medication instruction, pain and symptom control/hospice care

Supervises home health aide in compliance with the Medicare/Medicaid rules and regulations Performs other functions as required.

Qualifications

Associate Degree in Nursing Minimum one year in direct patient care setting or home health care License to practice as Registered Nurse in the state of New Mexico Current drivers license and auto insurance
