 Responsibilities: Drive the critical first step in a patients comfort journey by seamlessly bridging the gap between referral sources and specialized end-of-life care
In this vital dual role, you will use your clinical expertise to evaluate complex medical data, ensuring every patient is accurately matched to the perfect Hospice or Palliative Care program and level of care
It is a rewarding opportunity for a detail-oriented clinical coordinator who thrives on cross-functional collaboration and wants to make an immediate, meaningful impact on patient transitions Sign on and relocation bonuses are available for qualified candidates 🩺 Advanced Assessments & Direct Care Full-Spectrum Evaluations: Conduct holistic bedside assessments evaluating the physical, psychological/social, functional, financial, resource, and spiritual elements factoring into a patient’s condition
Symptom Management: Utilize expert nursing skills to continuously assess the presence of severe symptoms (e.g., pain, dyspnea, nausea, end-stage delirium) and measure response to comfort interventions
Acute Hospital Rounding: Assume the primary hospice RN role for hospital-admitted hospice patients, directing the initial plan of care and managing subsequent daily clinical rounding
Case Consultation & Intake Triage Goals of Care Mapping: Serve as the senior consultant to conduct critical conversations with patients and families regarding palliative philosophy, hospice criteria, preferred sites of care, and overall end-of-life goals
Referral & Resource Delegation: Review incoming referrals from all sources, determine patient clinical appropriateness, and strategically assign cases based on patient needs and team availability
Service Line Subject Matter Expert: Act as the definitive clinical resource for all multidisciplinary team members, maintaining deep expertise in all program offerings across the transitions service line

Associate’s degree in nursing New Mexico RN License required 1 year of hospice experience required 1 year of acute care experience is strongly preferred Current driver’s license and auto insurance BLS required Were all about well-being, starting with yours
