​ Saint Lukes Hospital is looking to add a RN, Care Coordinator to the Care Connections team. The ideal candidate will have previous care coordination or case management experience in an acute care setting. BSN Degree required within 3 years of start of position. Shift: Full time Days Job Description: Care Connections Nurse is part of a team designed to effectively assist patients in transitioning from hospital to community. Care Connections Nurse will work with the patient to focus on collaboration between hospital and community-based providers and arranging post discharge services. Care Connections Nurse functions as a member of a multidisciplinary team with a goal of improving the patient’s health status and reducing the need for hospital care.

Job Requirements Applicable Experience: 1 year Basic Life Support - American Heart Association or Red Cross, Registered Nurse - Various Bachelor's Degree - Science in Nursing Job Details Full Time Day (United States of America)
