  • Registered Nurse, Continuity of Care Clinic PRN About City of Hope , City of Hopes mission is to make hope a reality for all touched by cancer and diabetes. City of Hope’s growing national system includes its Los Angeles campus, a network of clinical care locations across Southern California, a new cancer center in Orange County, California, and cancer treatment centers and outpatient facilities in the Atlanta, Chicago and Phoenix areas. To demonstrate support of the hospital mission and vision by providing nursing, case management/disease management services to clients and families to help in the coordination and management of individuals’ health needs through the full continuum. The Registered Nurse demonstrates the ability to assess, coordinate, plan, monitor, implement and evaluate the patient’s plan of care while ensuring and facilitating the achievement of optimal quality, clinical and financial outcomes. Performs assessment of clinical and life issues for the patients of COH. Provides professional nursing services in support of the physician, in the direct and indirect care of patients. The successful candidate: Must be able to work PRN between Monday-Friday; 8a-4:30p. Provide oncology-focused nursing care, including disease management, symptom assessment, and monitoring for treatment-related side effects. Educate patients and families on diagnoses, treatment plans, symptom management, and quality-of-life concerns throughout the continuum of care. Perform comprehensive patient assessments, toxicity screenings, and timely documentation in the electronic medical record.
  • Collaborate closely with physicians and the interdisciplinary care team to coordinate and manage patient care. Triage patient concerns and clinical issues in person and by phone, intervening appropriately through standing orders, protocols, and provider consultation. Serve as a patient advocate and clinical resource, addressing health concerns and communicating significant findings to providers. Coordinate services between patients, families, providers, and external healthcare agencies, including home care services and medical equipment needs. Deliver patient and caregiver education using evidence-based resources and document all education provided. Promote an exceptional patient experience through responsive, compassionate, and patient-centered care. Support team operations by assisting with symptom management calls, patient inquiries, walk-ins, and coverage needs to ensure continuity of care and efficient service delivery.

Qualifications

Your qualifications should include: Current Georgia RN license from an accredited college BSN required or enrolled in a BSN program within 6 months of hire and completed within 3 years of date of hire BLS certification required.
